Tension Nose

Topic – Tension Nose – The term “Tension nose Rhinoplasty” is well known to many rhinoplastic surgeons, yet confusion exists with respect to its exact significance. Tension deformity is defined as excessive growth of the quadrilateral cartilage, overgrown in all directions: the top, the front, and the bottom. Correction of a tension nose requires careful reduction of each of the parts that are oversized in a proportionate fashion. The diagnosis of the tension nose is made by observing an overprojected dorsal septum and superior displacement and/or anteroinferior rotation of the tip cartilages. Correction of this deformity is obtained using a complete transfixion incision in association with a rim strip or lateral crural flap resection of excess tip cartilage. A transcartilaginous approach to the tip cartilages is advocated to preserve cartilage stability.
